What is Varicocele Surgery?

Varicocele occurs with abnormal dilation of the veins, especially in the left testicle. Among the symptoms of varicocele, pain, especially in the area where the veins dilate, with a decrease in testicular size due to heat in progressive enlarged varicoceles, hormone deceleration and sexual erection, which we call erectile dysfunction, may occur. The most common effect is a decrease in sperm quality due to the release of toxic substances due to this increase in temperature and dilation of the blood vessels. This condition causes infertility.

Varicocele is a disease that is treated only by surgical methods, but if there is no deterioration in sperm quality, varicocele is only followed. The current standard of varicocele is varicocelectomy surgery performed by microsurgery.

What is Varicocole Surgery?

Varicocelectomy is a surgical procedure to treat severe varicocele that is painful or affects fertility. A surgeon who specializes in disorders that affect the urinary and reproductive system cuts and seals the affected veins. This allows blood flow to other healthy veins in the scrotum.

How is a Varicocele diagnosed?

Varicocele is typically diagnosed through a physical exam and imaging tests. During a physical exam, the healthcare provider will examine the scrotum and testicles while the patient is standing up and lying down. The provider will look for swelling, lumps, or other abnormalities in the scrotum and testicles.

Imaging tests, such as a scrotal ultrasound or Doppler ultrasound, may also be used to diagnose a varicocele. These tests use high-frequency sound waves to create images of the blood flow in the veins and can identify any abnormalities or blockages in the veins.

Varicocele Surgery Methods

  • Microsurgical varicocelectomy (open surgery):

It is the most invasive of surgical approaches. The surgeon makes an incision in the groin and physically locates and ties the dysfunctional vein to prevent further blood flow.

  • Laparoscopic varicocelectomy:

Under general anesthesia, surgical instruments are inserted into the body through small incisions. Surgeons use special cameras to look inside the body, identify problem veins, and block further blood flow.

In addition to these surgeries, varicocele is treated with percutaneous embolization. This is a minimally invasive medical procedure used to treat a variety of conditions by blocking blood flow to certain areas of the body. It involves the insertion of a catheter through a small incision in the skin and guiding it to the target area using imaging guidance, such as X-ray or ultrasound.

Before Varicocele Surgery

Before varicocele surgery, your doctor will perform a physical examination to confirm the presence of the varicocele and assess its severity. Your doctor may also order imaging tests, such as a scrotal ultrasound, to get a better view of the affected area. In addition to these procedures, your doctor may also perform blood tests to check your hormone levels, as varicoceles can sometimes affect testosterone production. Before the surgery, you will likely be asked to avoid food and drink for several hours to prepare for the anesthesia.

After Varicocele Surgery

After varicocele surgery, the affected veins in the scrotum should be repaired, and blood flow should return to normal. This can alleviate symptoms such as pain, discomfort, and infertility.

After the surgery, patients may experience some pain, swelling, or bruising in the scrotum, which typically subsides within a few days to a week. Patients are usually advised to avoid strenuous activity and heavy lifting for a few weeks after the procedure to allow for proper healing.

People can usually return to normal daily activities within 2-3 days. However, full recovery may take three weeks. There are several factors that cause recovery time after varicocelectomy to vary from person to person, including:

• The type of treatment received
• Age of the patient

If left untreated, varicocele can cause a range of symptoms, including pain, discomfort, and infertility, and may also lead to complications such as testicular atrophy (shrinkage) or blood clots. If you have been diagnosed with a varicocele, it is important to discuss your treatment options with your healthcare provider.

Yes, varicoceles can affect fertility in some men.

Varicoceles are generally not considered to be life-threatening. However, in very rare cases, a varicocele may be a symptom of an underlying medical condition, such as a tumor or other abnormal growth in the abdomen or pelvis.

If you have varicocele, you should avoid lifestyle choices such as excess refined carbohydrates, added sugars, canned or salty foods, alcohol and smoking.

If you have been diagnosed with a varicocele, there are a few things you may want to avoid to help manage your symptoms and reduce the risk of complications. These include:

  • Prolonged standing or sitting
  • Heavy lifting
  • Tight clothing
  • Hot baths or saunas

It’s important to note that while avoiding these things may help manage symptoms associated with varicoceles, it is not a substitute for medical treatment or advice from your healthcare provider.

If you notice any of the following signs and symptoms, it’s a good idea to see your urologist.

·       Burning sensation while urinating

·       When the force of urine is weak

·       Pain when urinating

·       Pain in the pelvis area

·       Urine leakage

·       Frequent urination

·       Scrotal lump

If you’re receiving general anesthesia, avoid eating or drinking 8-12 hours prior to varicocele surgery as this may increase the risk of vomiting and aspiration.

Varicocelectomy is moderately painful. The pain should improve after a few days. You can treat the pain with over-the-counter pain relievers.

In general, most people can return to work within 1-2 weeks after the surgery, but it may take longer if your work involves physical labor or strenuous activity.

There is no change in sexual function after either treatment. Sex is not painful if you wait the recommended time to heal.

The length of time you need to stay in Turkey for pyeloplasty will depend on the specific details of your procedure and your recovery. In general, varicocele surgery is typically performed on an outpatient basis, meaning you will likely be able to go home on the same day as your surgery. However, in some cases, your surgeon may recommend an overnight stay in the hospital for observation.

After varicocele surgery, it is recommended to sleep on your back with a pillow under your knees for the first few days. This position helps reduce tension on the surgical incision.
