What is Rhinoplasty?

Closed Rhinoplasty

Closed rhinoplasty is a procedure that requires many years of experience and dexterity, since the entire operation is performed in a narrow tunnel. In closed rhinoplasty, the nose is first photographed from eight different angles. After the evaluation of our doctor with the photographs obtained, your aesthetic wishes are listened to, and the operation is decided.

With closed rhinoplasty, the tip of the nose can be lifted, the bridge of the nose can be intervened or the asymmetries of the nostrils can be changed, but the structure of the nasal bones remains unchanged after the operation. In closed rhinoplasty, special tools are used for nasal bumps on the nose instead of any chisel or surgical hammer. The right and left sides of the septum are cut longitudinally, and two horizontal incisions are made on the upper part of the nose. The upper parts of the nostrils are also separated by a longitudinal incision. The previously removed bone grafts are then placed in the cuts on the septum. The horizontal graft is fixed with a few stitches to fit the nose shape. Narrow silicone wings are then placed inside the nose to fix the appearance of the nose.

In closed rhinoplasty, unlike open rhinoplasty, bruising and edema are much less in the healing phase. But because of it is performed without the use of any chisels or surgical hammers, there is no scar after the operation. Open Rhinoplasty

Unlike closed rhinoplasty, the open or external rhinoplasty approach uses a small bridging incision called a trans-columellar incision to connect the right and left nostril incisions. In exchange for this 4-5 mm visible section, the nose skin can be folded upwards (similar to opening a car hood) and unobstructed visibility of the lower nose skeleton can be achieved. In addition to direct visibility of nearly the entire nasal framework, distortion of the nasal cartilage is minimized and individual components can be evaluated in their natural, intact alignment. Therefore, the hallmark of open rhinoplasty is the greatly improved surgical access allowed by the transcolumellar incision.

Although many aspects of an unattractive nose can be corrected using a closed rhinoplasty approach (thereby avoiding a trans-columellar incision), the modest risk of a visible columellar scar is well balanced by improved accuracy, versatility and effectiveness. While some rhinoplasty surgeons remain adept at the closed technique, especially in patients with relatively simple nasal anatomy, the advent of open rhinoplasty has truly revolutionized the treatment of complex nasal deformities such as cleft lip rhinoplasty or severe post-rhinoplasty deformities.

In fact, most rhinoplasty specialists consider open rhinoplasty surgery to be the procedure of choice for all types of difficult nasal anatomy. However, the technical demands of open rhinoplasty are rigorous and significant dedication and commitment to this approach is required to consistently achieve superior results.

Piezo Rhinoplasty

Nasal aesthetic operations, in which the nasal bones are cut and filed with tiny saws that produce ultrasonic vibrations and have tissue selection feature, are called "Piezo Rhinoplasty" for short. Thanks to Piezo surgery, which is sensitive only to hard tissues such as bone, which provides very successful results when applied to selected patients; Since the vibration strength can be adjusted according to the bone density, the bones can be cut and shaped very precisely. Therefore, the Piezo technique offers high safety and success in patients with thin bones. Since the vessels, tissues, cartilage, nerves or muscles surrounding the bones are not intervened, the comfort of the patients increases in the post-operative period. The nasal septum can be worked more precisely and the healing process of the patients is shortened as the soft tissues are not damaged. Since the area to be treated is opened, it is possible for surgeons to see the area more easily and it is possible to create a safe surgical area. Accordingly, complaints such as bruising and bleeding are minimized. Both minimal and major structural changes can be made in the nose.

Piezo surgery can be applied to patients who are not satisfied with the shape of their nose, who have nasal congestion problems in addition to their nose shape problems, and whose general health condition is not a problem for the surgery. Septoplasty

Nose surgeries are the first aesthetic operations that come to mind, but septoplasty, which is also called functional rhinoplasty, which improves the quality of the life of the patients by improving all functions of the nose, is at least as popular as nasal aesthetic procedures. In septoplasty procedure, an operation related to the external appearance of the nose is not performed. Septoplasty is performed to eliminate cartilage and bone curvature problems that cause breathing problems in the nose. The purpose of this surgery is not to create or provide a natural and beautiful nose to the patient aesthetically, but to ensure that the patients breathe in a healthy way. It can be done with the chronic sinusitis and sleep apnea operation.

Septoplasty is applied in the situation as follows:

Although Septoplasty can be performed with local anesthesia in some cases such as heart failure and hypertension, septoplasty is performed under general anesthesia. The operation time is about an hour. Usually, an invisible incision is made in the nostril, reaching the septum under the mucosa, and bone and cartilage are reshaped, thus correcting the curvature. If necessary, rasping or complete removal can be done. When the correction process is finished, the operation is terminated by placing a buffer (silicone or self-melting) suture in the nasal cavity.

Revision Rhinoplasty

Side effects/complications seen in the patient after the first rhinoplasty, damage and deformations in the nose, tissues, and face area, the appearance of the patient different from the planned due to incorrect application, the patient's dissatisfaction with the new appearance the most important factors in deciding for revision rhinoplasty.   • Continuation of the ugly appearance as a result of insufficient removal of the arch in the nasal dorsal bone or excessive filing / carving,
  • Nasal dorsal bone and cartilage structure collapses,
  • Inability to correct the curvature of the nose after rhinoplasty (deviation - twisted nose),
  • Failure to correct the nasal tip drooping or the occurrence of drooping nasal tip after rhinoplasty,
  • Opening and pit formation in the nasal dorsal bone as a result of the incomplete fracture of the nasal bones from the sides,
  • Asymmetrically excessively small or excessively large nostrils,
  • Lifting the tip of the nose more than necessary after the intervention
  • Excessive scarring or skin and soft tissue problems in the inner or outer region of the nose,
  • It can be seen that the new size of the nose is disproportionate to the face and asymmetry occurs.

To eliminate these problems, revision rhinoplasty is applied.

Rhinoplasty for Men

One of the most important reasons for the need for male rhinoplasty surgery is that men experience nasal breathing difficulties than women. Experienced nasal traumas can disrupt respiratory function as well as the external appearance of the nose, and both functional and aesthetic corrections should be made together in its treatment. Before Rhinoplasty Operation

Before the rhinoplasty surgery, a detailed physical examination is performed after listening to the patient's wishes and complaints during the interview with the patient. In this examination, the patient's nose is evaluated both in terms of aesthetics and in terms of breathing problems. Because breathing problems can be corrected by plastic surgeons in the same surgery. A detailed facial analysis is performed by taking pictures of the patient in terms of aesthetics, and the nose that will go to the face is determined. Before the rhinoplasty, a simulation study is applied to make the most suitable nose shape for the patient's face. When you take the day for your surgery, what you did or didn’t are very important. After Rhinoplasty Operation

Contrary to the general belief, rhinoplasty operations do not cause very painful and difficult post-operative periods. Of course, there will be some pain. The pain, which may vary according to the age of the person, the techniques used in the surgery, and the interventions made to the patient's bone and cartilage structure, usually alleviates significantly within 72 hours. Early pain can be managed with painkillers.

After rhinoplasty operations, edema and bruising usually go away within 10-15 days, and patients can clearly notice the change in the shape of their noses. However, it may take up to 12 months for the edema to completely disappear and the nose to take its final shape. During this period, edema gradually decreases over time and is usually only noticeable by surgeons.

When it comes to scars, it is usually one of the most concerned issues whether there will be any scars after rhinoplasty. Since no incision is made on the nasal skin during closed rhinoplasty and all interventions are performed through the nostrils, patients do not experience scar problems. In the open technique, scar formation occurs because the "columella" between the nostrils is cut. Although it is obvious at the beginning, this scar fades away during the healing process and there is no aesthetic problem.

Sometimes the patient would not be happy about the final results and this may pave the way to the revision rhinoplasty. In order to avoid the need for revision after rhinoplasty, surgeons evaluate all the variables before the operation. However, factors such as the patient's skin structure, age, genetic diseases, bone structure, and whether it complies with what needs to be considered during the recovery process may reveal the need for revision. Like any surgery, it cannot be guaranteed that complications will not occur after rhinoplasty. Revision rhinoplasty may be required even after rhinoplasty operations performed by the most experienced, successful and skilled surgeons.

In the first week after rhinoplasty, one should lie down with the head elevated. In order to minimize swelling in the early period, gentle cold compresses should be applied 4 to 6 times a day. The nose should be protected from impacts. No smoking should be allowed for 2 weeks after the operation. (Smoking should be stopped in the 1-week period before the operation.) Rhinoplasty can be performed in four seasons; the important thing is to avoid the sun. Precautions should be taken to protect from sun rays during the 6-month period after rhinoplasty. Glasses should not be used until the 3-month period after rhinoplasty is completed. Tough exercises that will increase blood pressure should be avoided for a minimum of 3 weeks. Air travel should not be done for 7-10 days after rhinoplasty.
